  • Health History
  • Hearing test – forced whisper (audio meter testing available upon request)
  • Physical examination
  • Height and Weight
  • Vitals (blood pressure, pulse)
  • Urinalysis (glucose, blood, protein)
  • Vision Test
  • Acuity- far and near vision test
  • Horizontal field
  • Color- Ishihara 14 plate assessment
